Sioux Falls, S.D.-based Sanford Health has expanded its remote patient monitoring service to support patients with chronic conditions through text-based symptom tracking.
The free program enables patients to self-report symptoms related to conditions such as chronic obstructive pulmonary disease, blood sugar, breathing, heart health and mental health. Patients do not need an app or device, and they can submit responses via text message in just a few minutes each week.
The service increases the frequency of communication between patients and care teams, enabling earlier interventions, according to an Oct. 7 news release. Sanford Health has reported fewer emergency department visits and fewer hospital stays outside of scheduled appointments among participating patients.
Lindsay Daniels, RN, Sanford Health’s executive director of care management, said the program also reduces barriers to care for patients with travel challenges, while Andrew Larson, director of operations for primary care, said it allows patients to share real-time data with trained clinical staff who can respond accordingly.
